Insurance Coverage Verification:

At Forward Chiropractic, we ensure that you have a complete understanding of the costs associated with any assessments or treatments in our clinic. We will gather your insurance information and verify your benefits with your insurance company. This will allow us to determine if you have chiropractic coverage, the number of visits covered, copay/coinsurance, and your annual deductible. Please allow us one business day for verification.

 

Understanding Your Deductible:

A deductible is an amount you must pay out of pocket before your insurance plan covers certain services. Deductibles can vary by plan and reset annually. We will verify if your plan has a deductible and, if so, inform you about the in-network rates for services covered by your insurance.

 

Insurance Referral Requirement:

You do not need a referral to visit our practitioners, but if you have an HMO plan, this may require a referral. We will gladly verify this for you before your first appointment.

 

Uninsured Care Options:

In case your insurance does not cover chiropractic care, we offer special pricing for all services, so you can still access the care you need.

At Forward Chiropractic, we operate on a pay-per-service basis. We do not require or offer pre-payment plans for our services as we prioritize providing the highest level of care for our patients without any binding or long-term commitments.
